Proof of Individual Identity (Name and Date of Birth)

As an individual entity, you must provide at least one primary source document or two secondary source documents, as listed below, to satisfy proof of identity.

Non-US citizens with visiting visa classifications are not eligible to obtain a DC driver license or DC identification card. Non-US citizens may obtain a DC driver license or DC identification card if they possess a USCIS Notice of Action, Form I-797, reflecting you have applied for an employment authorization card, permanent resident card or adjustment of status

If you have a name change due to marriage or a court order, you are also required to bring (in addition to the above documents) your marriage certificate or court documents to show proof of your name change.

Primary Sources (one required) - must be original:
  • State-issued United States birth certificate or birth certification card
  • DC driver license, learner permit, or identification card, not expired for more than 180 days
  • Unexpired United States passport
  • Certificate of Naturalization
  • Certificate of US Citizenship
  • Letter with picture from Court Services and Offender Supervision Agency (CSOSA) or DC Department of Corrections certifying name and date of birth
  • Unexpired Foreign Passport w/visa and I-94 reflecting duration of stay
  • Form I-94 (Arrival and Departure Record) stamped Asylee, Refugee, or Asylum Approval Letter
  • Unexpired Foreign Passport w/visa (classification A and G), I-94, and US State Department Letter (reflecting no diplomatic immunity)
  • Unexpired Foreign Passport w/visa (classification F), I-94 and I-20
  • Unexpired Foreign Passport w/visa (classification J), I-94 and DS-2019
  • Unexpired Employment Authorization Card
  • Unexpired Permanent Resident Card
  • Unexpired Permanent Resident Alien Card
  • Unexpired Refugee Travel Document
Secondary Sources (two required) - must be original:
  • DC driver license, learner permit, or identification card, which has not been expired for more than 365 days.
  • Certified copy of high school or college records from a United States jurisdiction or territory reflecting applicant's full name and date of birth.
  • Unexpired United States military identification card or discharge papers (DD-214).
  • Unexpired health insurance card reflecting applicant's full name and date of birth.
  • Unexpired identification card issued by a government agency reflecting the expiration date and the applicant's full name, date of birth, and photo.

Note: DC driver license, learner permit or identification cards for non-US citizens will expire on the US Immigration expiration date. Therefore immigration documentation must be resubmitted upon renewal.

Proof of US Citizenship (Commercial Driver License)

  • Valid, unexpired US Passport
  • Certified copy of birth certificate filed with a State Office of Vital Statistics
  • Consular Report of Birth Abroad (CRBA) issued by the US Dept of State
  • Certificate of Naturalization issued by the US Dept of Homeland Security
  • Certificate of Citizenship issued by the US Dept of Homeland Security

Proof of Lawful Permanent Residency (Commercial Driver License)

  • Valid, unexpired Permanent Resident Card issued by USCIS or INS

Proof of Business Identity

As a business entity, you must provide at least one primary source document, as listed below, to satisfy proof of identity.

Primary Sources (one required) - must be original:

  • DC Business License.
  • Certificate of Occupancy.
  • Government issued document showing DC Tax Identification Number.
