| One-Week permits are issued to customers who only require the placard for a week or less, such as visitors, residents with a short-term disability or residents whose vehicle with disability tags is being repaired. The customer can self-certify their disability without a requirement to obtain a physician's certification. The permit is not renewable within a one-year period. |
Temporary placards are issued to customers with a physical condition that substantially impairs their mobility, and is expected to last no longer than five years. Your physician must certify the application and indicate the expected length of your disability. If your disabled privileges are needed for a longer period of time, a new application with a physician's certification is required. |
Long-term placards are issued to customers with a long-term disability. The list of qualifying disabilities are included on the application. A physician's certification is not required if the customer has an obvious missing lower extremity. The placard is valid up to 8 years to coincide with the expiration of the customer's license or identification card. To renew, a new application with a physician's certification is required. |
